Children and young people with complex needs arising from a disability may attend or be on the waiting list for a Children’s Disability Network Team. If they need to be transferred to another Children’s Disability Network, for instance due to a change of address, this policy ensures the transfer is conducted in a safe, coordinated, equitable, and family-centred manner. The policy includes a clinical transfer form in the appendix.
