HSE National Clinical Guideline

Prevention and Management of Venous Thromboembolism in Pregnancy

Topic: Prevention and management of venous thromboembolism in pregnancy
Document Owner: National Clinical Lead for Guideline Development in Maternity and Gynaecology Services, National Women and Infants Health Programme (NWIHP)
National Group: Guideline Development Programme, National Women and Infants Health Programme
Effective From: 21/10/2026
This Guideline aims to provide evidence-based recommendations for the care of women at risk or diagnosed with VTE as well as promoting a standardised approach nationally across all maternity units.

This national clinical practice guideline establishes evidence-informed recommendations for the prevention, risk assessment, diagnosis, and management of venous thromboembolism (VTE) during pregnancy and the postpartum period. It advocates systematic and repeated risk evaluation, prophylactic low molecular weight heparin for individuals with major risk factors or elevated cumulative risk scores, and specialist-led anticoagulation management for those with prior VTE or significant thrombophilia.
