Open Disclosure Themed Week 2025

Open Disclosure Themed Week takes place from Monday 6 to Sunday 12 October 2025.

The purpose of this week is to raise awareness of the importance of open disclosure for staff, patients and service-users, their support persons and caregivers. Everyone has a crucial role to play in open disclosure and in improving patient safety.

This year aims to increase staff and public awareness of the importance of a culture of openness and honesty that is fair and just, A culture that focuses on learning and quality improvement following patient safety and notifiable incidents.

HSE Open Disclosure Policy 2025

Launched in June 2025, six years since the last revision. The revised policy follows and incorporates new national developments. In particular the Patient Safety (Notifiable Incident and Open Disclosure) Act 2023 and the Department of Health National Open Disclosure Framework. Much learning a has been gathered over that time which has driven improvements that are timely and necessary. This week is an opportunity to promote the revised policy, and to support staff with its implementation.

How can you get involved?

Services across the country will be delivering a number of activities to promote the week and support staff with this important work.

The HSE National Open Disclosure team have developed a series of resources. These resources can be used to support you and activities planned in your service. The activities will help raise awareness for the importance of open disclosure.

Promoting and facilitating training on open disclosure is key for services also. Themed Week is an opportunity for services to ensure staff are compliant with mandatory training.

Tuesday 7 October
