National Quality and Patient Safety 2025 Annual Report

The HSE National Quality and Patient Safety (NQPS) annual report highlights the work undertaken across our health and social care services in 2025 to:

  • improve quality
  • strengthen patient safety
  • support a culture of learning and continuous improvement

The report outlines the activities in 2025 that align to the commitments in the Patient Safety Strategy 2019 to 2024. These commitments are:

NQPS continued to advance our commitments to quality and patient safety by working in partnership with patients, service users, families, carers, and staff.

In 2025 we have:

  • strengthened patient involvement
  • expanded education and learning opportunities
  • enhanced our response to patient safety risks including programmes of work in relation to the common causes of harm
  • improved the use of information and data
  • reinforced leadership and governance arrangements across the health service

Highlights from the 2025 NQPS Annual Report

Empowering and Engaging Patients

  • Published the revised Open Disclosure Policy 2025
  • Translated the 'My Medicines List' into over 50 languages
  • Engaged and partnered with patients across a number of workstreams

Empowering and Engaging Staff

  • Launched the QPS Competency Navigator
  • Published the annual NQPS Prospectus of Education and Learning programmes
  • Hosted 4 patient safety community events

Anticipating and Responding to Risks to Patient Safety

  • Published 6 Patient Safety Spotlight Series'
  • Updated National Sepsis guidelines and eLearning programmes
  • IRESTORE Residential Older Persons Early Warning System

Reducing Common Causes of Harm

  • Development of the Wound Management Clinical Resources packs
  • Published the Lithium Patient Information booklet
  • Continued the roll out of the QUICKPatientSafety App

Using Information to Improve Patient Safety

  • Launched the Clinical Audit Portal
  • Hosted Open Disclosure Themed Week
  • Shared a patient safety infographic on choking incidents in the HSE and HSE funded services

Leadership and Governance to Improve Patient Safety

  • Undertook extensive consultation and work to develop the revised Incident Management Framework
  • Established the National QPS Committee chaired by the HSE Chief Clinical Officer
  • Re-established the Irish Clinical Audit Network (ICAN)
