This HSE Procedure applies to all HSE Regional Health Areas (RHA) PHCP governance groups when a nursing service is delivered in a community setting to a child receiving a HSE Paediatric Homecare Package.

This procedure outlines the arrangements, roles, responsibilities, and communication pathways required when a child with complex healthcare needs who is receiving a Paediatric Homecare Package requires an urgent transfer to hospital.

This document is intended to ensure that children receiving a paediatric homecare package are transferred to hospital in a timely, safe, and coordinated manner during episodes of clinical deterioration, acute illness, medical emergency, or any other circumstance requiring assessment or treatment in an acute paediatric setting. It provides guidance for homecare staff and community healthcare teams on how to manage urgent transfers while maintaining continuity of care.

It is the policy of the HSE to ensure that children with complex healthcare needs who receive a PHCP are transferred safely and promptly to hospital when urgent admission is required because of clinical deterioration.
